Prescribers

This page is for Prescribers only and you will need a password to access some areas of this page such as recycled equipment and Pilot Forms (see below for how to obtain a password).

There is also access to a pool of recycled equipment that is available throughout NSW.  Call EnableNSW for any questions relating to accessing recycled equipment.

New pilot forms and guidelines for Orthotics and the Home Respiratory Program are available under Pilot Forms, these will be accessibile to everyone once all the prescriber training has been completed. 

Other forms for equipment and artificial limbs are available under Application and Other Forms.

Email EnableNSW if you do not have the password.  Please put Enable Website Password in the subject header.  Email requests should be sent from your work email not from a generic email account such as Hotmail or Yahoo.
