Do I need to conduct trials before requesting stock equipment?

No,as most of the stock equipment is standard off the shelf, no trial is required.

Can I request equipment for trial?

You may request 2 slings or 2 cushions for trial.

However,the item not required must be returned to the EAP. The Local Health District (LHD) is charged for both items if the unsuitable item is not returned to the EAP. The increased costs associated with these charges negatively impact waiting times.

What happens if the stockequipment is not suitable?

If the equipment from stock is not suitable, the prescriber should email with subject heading titled: person’s name and date of birth.

The email should specify the:

  • Replacement item from stock with the relevant part number, or
  • Request to pick up the unsuitable equipment.

How do I request purchased equipment as well as stock equipment?

Complete a separate ERF at for the items to be purchased and email this ERF to
