This indicator is intended as a guide to your eligibility to access EnableNSW Programs. It does not consider your individual circumstances. We encourage you to speak to a Customer Service Officer to discuss your individual circumstances and requirements.
Residential Aged Care Facility (RACF)
Do you live in a RACF (including Nursing Home or Hostel)? This may include short-term respite
Outcome
You are eligible to apply for the following EnableNSW programs:
- Prosthetic Limb Service (PLS)
- Equipment Allocation Program for a power wheelchair (PWC) if you use it on your own

Accomodation
Please select from the following options:
- You live in your own home (rent, own or Housing NSW)
- You live in a group home/residential facility operated by NGO
- You are currently in a NSW public hospital and due to be discharged to your own home/group home
- You are currently in a NSW public hospital with no discharge date or destination
- You are currently an inpatient in a private hospital
Outcome
You are not eligible for EnableNSW programs, but may be considered under exceptional circumstances.
Assistance from other programs
Please select from the following options:
- No Assistance - You currently do not receive any assistance from any Government funded programs AND you have not received compensation relating to your disability / condition
- Community Home Support Program (CHSP) - You are receiving assistance through a CHSP (formerly known as a HACC)
- Department of Veteran's Affairs (DVA) - You are eligible to receive assistance through DVA
- Dust Diseases Board *may be known as ICare - You are receiving assistance through this scheme for an asbestos related health condition
- Compensation - You have received compensation relating to your disability / condition. This can include *LTCSA, *Work Cover or *Third Party Insurance * May be known as ICare
- Australian Government Home Care Package (HCP level 1-4) - You have had an ACAT assessment and are on the National Queue waiting on a home care package OR you have had an ACAT assessment and are on the National Queue, however you are not actively seeking services at this time (this means you have removed yourself from the National Queue) OR you are receiving a Home Care Package or have accepted an interim package, while waiting on a higher level package?

Exceptional Circumstances
In exceptional circumstances discretion may be exercised by the Manager, EnableNSW to approve a request where:
- the person is not eligible for EnableNSW
- there is a lengthy delay in funding
- there is demonstrable health impact
- other funding sources have been exhausted
- the request meets the EnableNSW clinical criteria

- compensation was awarded for a different disability / condition
- in cases of demonstrable financial hardship

While you are inpatient in a private hospital you are not eligible for assistance through EnableNSW. Please assess your eligibility once you have been discharged to your own home/group home within the community.
