We’re making it easier for people on TCP to access equipment sooner!

From 1 July 2022, the EnableNSW team will now accept equipment requests for people on the Transition Care Program (TCP), four weeks before their package ends.

This means that requested items from the Equipment Allocation Program (EAP) may be delivered before the person’s treatment is finished.

Clinicians who are prescribing equipment for people on a TCP package are asked to provide the following information with the application:

  • the date the package is due to end for the person i.e. their discharge date
  • confirmation the person will not be eligible for, or commencing a My Aged Care Home Care Package
  • confirmation the person is remaining in their home in the community
  • confirmation the person will not be moving to a residential care facility.

This information, together with a complete application form and equipment request form are required for the team to process the request and provide an outcome.
