Application process

Step 1:

Check your eligibility using the online eligibility checker

Step 2:

Contact the private dialysis unit(s) you are planning to travel to and check availability for your planned travel dates/sessions. Visit the participating dialysis units page to find out where the units are located in Australia, including their contact details

Step 3:

Complete the AFHH application form either digitally or manually. The information will be used to confirm eligibility

Step 4:

Your preferred travel dates will be sent to the nominated dialysis unit(s). If your travel dates have been accepted, the EnableNSW AFHH customer service team will send you a confirmation letter

Step 5:

The receiving dialysis unit(s) will contact you directly to discuss the criteria you need to meet before you arrive for your scheduled appointment
