The application process requires submission of the SEED Application template email with an EnableNSW Application Form. EnableNSW will review the SEED application and advise the applicant and clinical service in writing of the outcome of the application.
If approved, prescribers from the clinical services can then submit equipment requests:
- For stock equipment available through the Equipment Allocation Program (EAP), mark the Stock Equipment Request Form as SEED
- For other contract equipment, mark the Equipment Request Form as SEED and include the contract codes found on the Quick Reference Guides
- For equipment not available in stock or on Government contract, the prescriber should contact the EnableNSW Clinical Advisor to discuss other options
